Accessible Health Action Plans are created during a GP’s annual health checks with people with learning disabilities and autism.
The action plan is designed to ensure people are taking appropriate steps to remain in good health and prevent future health issues.
How Brandon has supported creating an accessible Health Action Plan
Bristol, North Somerset and South Gloucestershire Integrated Care Board (BNSSG ICB) commissioned Brandon’s Involvement Team to work with service users to develop a more accessible and easy-to-use Health Action Plan for GPs in the region.
The group, including Brandon’s Adventurers were facilitated to develop ideas and then worked with a designer to develop the concept into a single page per patient.
The new Health Action Plan co-produced with Brandon’s Adventurers has now been distributed to all GP practices in the region and will potentially benefit more than 5,000 people with learning disabilities across BNSSG when they have their Annual Health Check in the coming months.
BNSSG ICB will also be sharing the plan with other NHS providers across the South West region, for the benefit of thousands more people with learning disabilities and autism.
