The Brandon team in Roche, Cornwall, has won the Shared Decision Making Award at the South West Integrated Personalised Care Awards 2023.
The awards event, held at Dartington Hall in Devon before Christmas, saw the Brandon Roche team beat more than 100 other nominees in the Shared Decision Making category.
What is personalised care?
As explained on the NHS England website, “The purpose of personalised care is to empower people to lead the lives they want to live.
“Personalised care starts with a conversation with people about what matters to them. It builds on what people can do, as well as addressing their health needs.
“So at its heart personalised care is about people, and getting the right support to them in the place where they live. For personalised care to work for people it has to be integrated with all organisations working as a multi-disciplinary team…”
The Shared Decision Making Award
The South West Integrated Personalised Care Awards guidance for the award categories, describes how nominations for this category should demonstrate shared decision making in practice and the difference it made:
“No decision about me, without me! I am able to make informed choices and be at the centre of discussions about my health and care.”
Why the Brandon team was nominated
Nominated by the NHS, the award was presented at the event on 12 December for the collaborative partnership between Brandon, Cornwall NHS Foundation Trust, and SwallowCare Group.
Together, they provided a successful transition for a vulnerable individual who moved from a group home to a supported living place.
The move to supported living has transformed the individual’s sense of wellbeing and she is now thriving in her new home. In the months since moving, the individual has become more accepting of support, is showing reduced anxieties, and living the life she wants with person-centred support from the team.
Collaboration achieves amazing results
Brandon locality manager, Joshua Vickers, said:
“It is a tremendous privilege to see what the team has achieved with all of the people they support. The culture within the team is infectious, with each member bringing their own skills and experience to everything they do.
“To win the award for the work we do with our partners, is recognition not only of the success seen with one individual, but also of the whole team and their willingness to work collaboratively with each other and our wonderful partners in the NHS to achieve amazing results for those we support.”
